Dr. Natarajan accepts most private insurances, self-pay and Medicare. Please call the office at 312.616.7778 to see if we accept your plan. If you are covered in a plan that we accept, we will directly bill your insurance for you. As the cardholder of your policy, it is your responsibility to check if the provider is under your plan. We currently accept:
- Aetna US Healthcare HMO, POS, PPO Health Plans
- Blue Cross Blue Shield HMO with referral, POS, and PPO Health Plans
- Cigna PPO
- CCN – Community Care Network PPO
- First Health PPO (formerly known as Affordable)
- Health Network PPO
- Humana HMO, POS, and PPO Health Plans
- PHCS- Private Healthcare Systems PPO and EPO
- The Preferred Plan PPO and EPO
- Unicare Health Plans (formerly known as Prudential Health Plans)
- United Healthcare of Illinois
If we do not accept your current insurance plan and you still want to see either Dr. Natarajan, you may do so by paying for your visits and then accepting the “out of network” reimbursement you receive from your insurance company. In most cases, we can work with you on payment arrangements.
HMO: Patients with HMO plans who wish to be seen by our physicians will require a referral from their primary care physician. They will be seen as sports medicine patients only.
Patients with medicare who wish to be seen by our physicians will require a referral from their primary care physician. Referring providers can fax the referral to (312) 276-4304. The referral must be present at the time of visit in order to be examined by the physicians.
Patients with Worker’s Compensation who wish to be seen by our physicians will require the following:
- Name of Claims Adjuster
- Claim Number
- Telephone of Claims Adjuster
- Fax of Claims Adjuster
- Claims Mailing Address
Third Party Liability Insurance
Patients with Third Party Liability Insurance who wish to be seen by our physicians will need to pay at the time of their visit. You can apply for reimbursement with your insurance. We can provide notes, records or receipts as needed to obtain reimbursement. Examples of incidents that would use Third Party Liability Insurance include but are not limited to: Motor vehicle accidents and Slip and Fall accidents.
